Database Questionnaire

The database questionnaire is completed by the customer and used to determine the general requirements of the handset users. The Questionnaire provides the Project Manager with information on the following aspects:

• Handset user names and handset types
• Class of service and restriction levels
• Location and CAT5 outlet numbers
• Manager/Secretary pairings
• Coverage paths and pickup groups
• DDI numbers/quantities
• Trunk numbers/quantities
